Can you have uterine cancer with a normal Pap smear?

Very, very occasionally it is diagnosed in a routine Pap smear, but Pap smears are designed to detect cervical cancer and do not reliably detect uterine or endometrial cancer, although occasionally the shed uterine cancer cells can show up in the Pap smear.

What tests determine uterine cancer?

An endometrial biopsy is the most commonly used test for endometrial cancer and is very accurate in postmenopausal women. It can be done in the doctor’s office. A very thin, flexible tube is put into the uterus through the cervix. Then, using suction, a small amount of endometrium is removed through the tube.

For a better view of the inside of your uterus, your doctor may recommend a transvaginal ultrasound (TVUS). In this case, the transducer gets close-up images from inside your vagina. Your doctor can look for a mass (tumor) or see if the endometrium is thicker than usual, which can signal endometrial cancer.

Can you feel uterine cancer?

When they’re first diagnosed, about 10\% of women with uterine sarcomas have pelvic pain and/or a mass (tumor) that can be felt. You or your doctor may be able to feel the mass in your uterus, or you might have a feeling of fullness in your belly and/or pelvis.

How often is uterine cancer missed?

Uterine cancer tumors can be visible on a transvaginal ultrasound. However, just as with any medical imaging tool the ultrasound results are imperfect and reliant on subjective interpretation by the doctor. According to an ACOG study, ultrasound fails to diagnose uterine cancer in about 1 in 239 cases.
