Why is rebar always rusted?
Not sure if you have noticed, but rebar tends to have ribs along the bar. The purpose of this is for a friction bond. Slight rust on the surface of the rebar adds to the friction resistance of the steel and concrete interface.
Can you pour concrete on rusted rebar?
Having rust on the surface of the rebar could cause further corrosion of the reinforcements. Further, placing concrete with the rust in the reinforcement, lose the bond between the concrete and reinforcement bar. It could lead to reducing the bond strength. As we know, concrete gets cracked due to durability issues.

One of the main sources of corrosion to rebar is salt. Whether through exposure to saltwater near a marine environment or application of deicing salts to make roadways safer during the winter, these chloride ions can make their way through the concrete, corroding the steel reinforcement.

Put the rebar on top of a raised non-metallic platform, such as wooden pallets. Both the ground and concrete areas absorb moisture and can lead to rusting. Cover the rebar with a thick protective tarp and secure it with cinder blocks (or something similarly heavy) at the corners and edges.
