Sumatra PDF is the only application I have found that does this well:

  1. Open the PDF, to see what it is.
  2. Hit F2 (inside Sumatra PDF, while you’re still looking at the PDF)
  3. File selector dialog comes up, with title “Rename to”
  4. Type new name of file, hit Save.
